Transitioning from pediatric to adult clinic is referred to as Health care transition.听 Health care transition is the process of a moving from pediatric to adult clinic with or without transferring to a new provider. It involves planning, transfer, and integration into adult-centered health care.听听

The goals of health care transition are:听听

1) To improve the ability of youth and young adults to manage their own health care needs and effectively use health services,听

2) To provide a process in pediatric and adult health care practices prepare for this transition.听
