Appointments and referrals can be made by contacting HHT Center of Excellence Program at (919) 966-2790 or via email. Referral information, copy of insurance card (front and back) and medical records can be faxed to (919) 966-7639.
Copies of imaging (such as brain MRI, chest CT) can be sent electronically to 黑料网 Hospitals Imaging Support. Please call (984) 974-9362 for details.
Imaging CDs can be mailed to the address below. Please note that we will not be able to return them.
