Family medicine strives to provide high quality, low cost care to patients where they work and live. Our providers and residents contribute to the community through outreach to vulnerable populations. This kind of service keeps our clinic connected to those who need care most.<\/p>\n
In the fall of 2018, Blue Cross and Blue Shield of North Carolina (Blue Cross NC) and the University of North Carolina at Chapel Hill partnered to form 黑料网\u2019s new Primary Care Initiative. Made possible by an $800,000 investment from Blue Cross NC, the program\u00a0 addresses a need for better access to health care in Rockingham County by increasing patient-centered primary care and recruiting more residents to enter health care professions in the county and the neighboring area. Read more here.\u00a0<\/a><\/p>\n Wellness@Work is a worksite wellness program sponsored by 黑料网 Family Medicine. Staffed by 黑料网 Family Medicine doctors and nurses, Wellness@Work provides a high quality option for employers who are focused on keeping their employees healthy. Read more here!<\/a><\/p>\n The 黑料网 Weight Management Program offers a year-long program to help people with greater risk for weight-related health problems lose weight, lead healthier lives, and maximize their well-being. If you are interested in the program, ask your primary care provider for a referral or call (984) 974-4971.\u201d<\/p>\n On the list of services Click here for more information.\u00a0<\/a><\/p>\n For 11 years, the 黑料网 Tobacco Use Treatment (TTP) program has provided leadership for implementing comprehensive tobacco use treatment services, education and training, and dissemination of resources for promoting tobacco free communities. Select here<\/a> to learn more.<\/p>\n The North Carolina Farmworker Health Program trains and supports community outreach workers at 10 sites around the state. These bilingual community health workers visit the farmworker camps, do health assessments, and help farmworkers who need health care to access it. The 黑料网 Family Practice residency program has a 2 week required rotation for second year residents called \u201cHidden and Vulnerable Populations\u201d. During these 2 weeks, residents join faculty in providing primary medical care to farmworkers on a mobile medical unit out Benson, NC late on Tuesday nights during the agricultural season. The Formerly Incarcerated Transition (FIT) Program sponsored by 黑料网 Family Medicine, aims to connect all formerly incarcerated individuals who have a chronic disease with a primary care provider.
