If you are a new ADULT patient at one of our ENT clinic locations, please print and complete the following adult patient intake form prior to your first appointment: Adult Patient Form

If you are the parent or guardian of a child being seen at one of our pediatric ear, nose, and throat clinic locations, please print and complete the following pediatric patient intake form prior to your first appointment: Pediatric Patient Form

If you are unable to keep an appointment, please call 984-974-6484 as soon as possible to reschedule or cancel. We request at least 24 hours’ notice, and in consideration of others, those who repeatedly miss appointments without notice may be limited in their ability to schedule further appointments.
