Our team has the expertise and experience to treat adults and children with allergies by getting to the root of your ENT ailment. We offer a multifaceted approach beginning with understanding your symptoms, determining the correct route of testing, and then finally finding a treatment method that works for you. Our physicians, APPs, and nurses are trained in best ENT practices, to go past your surface symptoms and treating the core of your head and neck allergy trouble.

How to Make an Appointment

Welcome to the Division of Otolaryngic Allergy Services and thank you for choosing our Allergy/Rhinology specialists听for your care. We have听five ENT clinic locations听where our ENT physicians see patients.听The phone numbers to schedule appointments are:听

  • ENT at Carolina Crossing, ENT at Meadowmont,听Head and Neck Oncology at the听Medical Center, and Pediatric ENT at Raleigh:听984-974-6484

To save you time when you arrive for your initial visit at one of our ENT clinic locations, we have made the following new patient forms available for you to complete and bring to your first appointment. Click to download the correct patient form:

  • If you are a new ADULT patient at one of our ENT clinic locations, please print and complete the following adult patient intake form prior to your first appointment: Adult Patient Form.
  • If you are the parent or guardian of a child being seen at one of our pediatric ear, nose, and throat clinic locations, please print and complete the following pediatric patient intake form prior to your first appointment: Pediatric Patient Form.

If you are unable to keep an appointment, please call 984-974-6484 as soon as possible to reschedule or cancel. We request at least 24 hours’ notice, and in consideration of others, those who repeatedly miss appointments without notice may be limited in their ability to schedule further appointments.
